Registered Nurse - Homecare Dept. CAP/DA

Under the supervision and guidance of the Director of Homecare and the President, the RN conducts in-home needs assessments of elderly and/or disabled adult with multiple problems; works in conjunction with the Case Manager to determine the appropriate services for the beneficiary.

Essential Duties:

  • Conduct a Comprehensive Assessment to determine need for services;
  • Conduct Continued Needs Assessment (CNR) every 12 consecutive months on or by the third week, to continue the need for services provided;
  • Assesses and evaluate the effectiveness and makes recommendations for Plan of Care changes;
  • Update information in the development of departmental policies, procedures, goals and objectives;
  • Coordinates program services with other inter- and intra-agency service resources;
  • Assist with in house team to monitors the status of program service delivery to ensure compliance with established goals, procedures, policies and internal requirements;
  • Prepares required program, funding source and agency reports;
  • Demonstrate safety, including universal precautions and safe work practices, established fire/safety/disaster plans, risk management, and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs;
  • Maintain confidentiality of all data, including beneficiary and operations data;
  • Knowledgeable of beneficiary’s rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all beneficiaries in a safe, secure environment;
  • Adhere to all policies and procedures as defined by Resources for Seniors;
  • Maintain confidentiality at all times according to agency’s policy and procedures and HIPAA requirements.

Knowledge, Skills and Abilities:

  • The fundamental knowledge of the methods and principles of program development, administration and supervision; of accepted practices, principles and techniques in the field of human services and the applicable program concentration and of interviewing and assessment techniques;
  • Ability to develop positive working relationships with staff, beneficiaries and other members of the community; to coordinate staff roles in delivery of services; to meet deadlines, arrange priorities and coordinate daily work activities, and to develop, maintain, and utilize accurate records;
  • Knowledge of the framework, function, and organization of a home care program and administration and supervision;
  • Working knowledge of the principles and practice of nursing according to nursing practice act, agencies policies and procedures, and reimbursement guidelines;
  • The ability to communicate effectively verbally and in writing;
  • ” Efficient in Microsoft Office products.

Minimum Qualifications:

  • Current RN licensure in state of practice, successful completion of a nursing education from an approved school of nursing with two years of clinical nursing experience and at least one year of homecare nursing experience preferred;
  • 12 hours of continuing education annually;
  • CPR certification;
  • Clean driving record with valid driver license and/or have access to reliable transportation.

  • While performing the duties of this job, the employee is regularly required to sit, verbally communicate or hear;
  • The employee is occasionally required to walk, stand and reach with arms;
  • The employee must occasionally lift up to 25 lbs.

Work Environment:

The RN will conduct home or facility assessments as required or phone interviews as need indicates. The interview will be conducted with the beneficiary, caregivers, and/or facility staff. Office hours may be required in order to complete reports, assessments, paperwork, and/or staff meetings/team reviews. Scrubs or dress in accordance with the Resource’s for Seniors dress code is appropriate. Minimal physical contact with the beneficiary occurs during an assessment.

This position is a full-time, benefits-eligible, exempt position. Salary is dependent on experience. This position has working hours of 8 a.m. to 5 p.m. Monday-Friday.
