Community Alternatives Program for Disabled Adults

This is a Medicaid-funded program offering in-home care for those clients who might otherwise require nursing home care.

This form is intended to facilitate the referral process to the Medicaid waiver program CAP/DA.

For the CAP/DA program, the client must be (1)a resident of Wake County, (2) 18+ years of age, (3) at a nursing home level of care or at risk for nursing home placement related to care needs, and (4) eligible for Medicaid (if you are not eligible, client will have to meet a monthly deductible).
