This is a general term for several programs providing limited in-home services on a sliding-scale basis for Wake County residents who need help with personal care. Lengthy wait list.

This form is intended to facilitate the referral process for the In-Home Care Program.
For the cost-share programs, the client must be (1) a resident of Wake County, (2) 18+ years of age, and (3) have personal care needs, NOT just housekeeping/meal preparation.
