Service Coordination
The Service Coordinator, under the supervision of the Service Coordination Manager, assists seniors with supportive services which may be necessary to remain self-sufficient and live in their own community for as long as possible; establishes links with other local agencies and service providers; provides information about senior related community resources; serves as an advocate for senior adults in the community; provides emotional support and guidance to seniors and/or their families; coordinates various health/wellness/educational presentations for the apartment complex as needed; maintains departmental and agency reports as required and performs other duties as appropriate and requested.
Essential Functions
- Connecting seniors with appropriate community resources;
- Establishing links with other local agencies and service providers to offer wider variety of resources and programs to the senior adults in the community;
- Scheduling a diverse selection of health/wellness/educational programs at the apartment complex as needed;
- Attending continuing education sessions and other trainings/meetings as needed;
- Maintaining departmental, agency reports and other required administrative records on an ongoing basis;
- Performs other related duties as delegated by Service Coordination Manager.
Knowledge, Skills and Abilities Required to Perform Essential Duties
- Four-year degree in a social science field/equivalent experience considered;
- Knowledge of services/resources available within the community for seniors and disabled adults;
- Advanced user in Microsoft computer applications (Word, Excel, Outlook, Power Point) and ability to manipulate other databases;
- Clean driving record and ability to travel between multiple work locations as needed;
- Advanced knowledge of Medicare, Medicaid, and other benefits/entitlement programs is recommended;
- Requires the ability to perform addition, subtraction, multiplication and division; ability to calculate decimals and percentages; ability to perform mathematical operations with fractions;
- Maintain the integrity of confidential employment, client, and business information;
- Excellent organizational and time management skills;
- Ability to accurately record and deliver information, meet deadlines;
- Ability to maintain complete, organized, and accurate files for all assigned tasks and program areas;
- Balance priorities within and between assigned complexes;
- Extremely self-motivated and able to work independently;
- Ability to effectively communicate;
- Verbally and in writing with clients, property management, other professional staff, and the general public;
- Interact positively to the culture, traditions, lifestyles, language, and values of each individual, family, and community;
- Recognize and respond immediately to crisis situations;
- Ability to remain professional, courteous and helpful at all times;
- Ability to use departmental equipment and materials;
- Knowledge and compliance of Resources for Seniors, Inc. policies and procedures;
- Attends all meetings and trainings as required by supervisor.
Physical/Environmental Demands
- Ability to exert physical effort in light to moderate work involving lifting up to 50lbs of force occasionally, and/or up to 20lbs of force frequently to move objects by carrying, pushing, and pulling (including daily transport of laptop/rolling bag);
- Ability to work effectively and cooperatively in an office environment and travel to off-site locations in varying weather conditions;
- Normal routine involves potential exposure to animals/bug infestations, cigarette smoke, and other chemical and environmental hazards;
- Perform effectively in environments with frequent workload changes and competing demands.

This service coordination position is a full-time, exempt position working an average of 40 hours a week Monday-Friday.
