Our pledge regarding medical & other private information
We understand that private information regarding your health, finances, services or other identifying data is personal and should be protected throughout your involvement with our organization. We are committed to protecting such information about you. This notice pertains to all personal and privileged information (PPI) we collect through any of the ways we conduct business with you (over the phone, on our website, in electronic format, in writing or in person.) It is our commitment to:
Make sure that personal information that identifies you is kept private;
Maintain the security of information held in electronic or hard copy form;
Give you this notice of our legal duties and privacy practices;
Notify you of any breach of personal and privileged information (PPI.)

Your Rights Regarding Personal and Privileged Information About You
Right to inspect and copy PPI that we maintain. Requests should be made in writing to the Privacy Officer
Right to amend. If you feel that PPI about you is incorrect or incomplete, you may ask us to amend this information. Requests should be made in writing to the Privacy Officer.
Right to an accounting of disclosure. You have a right to know who we made your PPI available to, where such disclosure was made for any purpose other than treatment, payment, or service operations. Requests should be made in writing to the Privacy Officer.
Right to request restrictions. You have the right to request a restriction or limitation on the PPI we use or disclose about you. Requests should be made in writing to the Privacy Officer.
Right to Request Confidential Communications. You have the right to request that we communicate with you about personal or confidential matters in a certain way or at a certain location. Requests should be made in writing to the Privacy Officer or to any agent of our organization.
Right to a paper copy of this notice. You may ask us to give you a copy of this notice at any time. To obtain a copy, please contact the Privacy Officer or any agent of our organization.
Changes to this notice
We reserve the right to change this notice and our privacy policies. If we make such changes, the new terms and policies will apply to all Personal and Privileged Information that we currently have or receive in the future. The effective date can be found on the first page of the notice, and the current notice will be in our administrative offices or can be obtained by contacting the Privacy Officer.
Complaints?
If you believe your privacy rights have been violated, you may file a complaint with Resources for Seniors Inc. or with the Secretary of the Department of Health and Human Services. To file a complaint with Resources for Seniors Inc., contact the Privacy Officer. All complaints must be submitted in writing. You will not be penalized for filing a complaint.
