This form is intended to facilitate the referral process to the Adult Day Centers Funds Program.
You may print it, fill it out, and fax it to us, or you may fill it out below and it will be emailed to us.
For the Adult Day Center program, the client must be (1) a resident of Wake County, (2)18+ years of age, and (3) have personal care needs, NOT just housekeeping/meal preparation.
An Intake specialist, who can be reached by calling 919-713-1556, can assist you if you are unsure whether the client would be a good candidate for this program.
