Care Manager

KNOWLEDGE, SKILLS AND ABILITIES

  • Knowledge of the aging process and the needs of the older adult, of the resources available to meet those needs and of the methods
    used to access, arrange and maintain the successful delivery of services;
  • Demonstrate skills with in-home visit assessments, monitoring and coordination; conduct initial, on-going and follow-up interviews;
  • Ability to maintain documentation of work activities and to prepare reports;
  • Ability to develop positive working relationships with staff, clients and other members of the community;
  • Demonstrated time management skills to meet deadlines, arrange priorities and coordinate daily work activities of self;
  • Ability to meet physical demands of the Essential Duties with or without reasonable accommodations;
  • Ability to work cooperatively, patiently professionally with the older adult population and other diverse populations, their
    representatives, subcontractors and agency staff;
  • Strong interpersonal and communication skills with the ability to be a positive team player;
  • Adhere to all company policies, practices and procedures.

The ideal candidate would also have the following training and experience:

  • Bachelor’s degree in Social Work or related human service field equivalent;
  • One year experience in the provision of direct services, preferably in the field of aging, as a student or in a paid capacity; or
    equivalent work and experience;
  • Experience with community in the health or medical field directly related to homecare, long term, or personal care.

The position is a full time, benefits eligible, exempt position working within the hours of 8:00am-5:00pm, Monday-Friday.
