Community Alternatives Program for Disabled Adults:
This is a Medicaid-waiver program offering in-home care for those clients who might otherwise require nursing home care.
Wake Independence @ Home Care Program:
This is program provides limited in-home aides for Wake County residents who need help with personal care. Lengthy wait list.
Housing and Home Improvement:
Wheelchair ramps, grab bars and hot water heaters ONLY.
Adult Day Care Funding:
Funding to help Wake County clients pay for the cost of PARTICIPATING adult day care centers. Lengthy wait list.
Caregiver Support Program
Funding to help Wake County UNPAID caregivers receive incontinence products, nutritional supplements, or both, for a care-receivers age 60+. Can apply every 12 months from first shipment.
